How to do FOIPA request

I tried to do a FOIPA request through the website:

http://foia.fbi.gov/

Which form should i use, the FOIA Request or the Privacy Act request?

For FOIA request, do i need my request notarized? Should i enter a number for "Enter maximum amount you are willing to pay:"? Can't request online, is that correct?

I've sent the PA requests for my files to both, FBI and CIS on Friday, the 11th
FOIA - generally used for info regarding a third party - was advised not to use it

May i ask two questions?
1. What's the CIS address for the PA request? I thought only FBI can do that.

2. What's your last LUD about?
