How to change official names after GC?

Hi Folks,
My spouse's GC just got approved and we also received the plastic card. We want to change her official names (both first and last). Has anyone done that? I am not sure how to proceed and how to obtain a new green card from INS with the changed name?

Anyone who has done that or is familiar with the process please post.
