How to change address in CSC? Please help.

I did my Passport Stamping today. I told the officer I will move soon.
He gave me a form. The foem said only used for I90/I485/I-751/I-765.
No others. BTW, I was in San Jose INS office. The foem is for CSC only.
I heard we need to fill out AR-11. Why INS give me a different form?
Which one should I fill out? Thanks for your help.

The form have no number, just have some words: CSCand Address Change Form on the top.
