How to apply reentry permitt

The I131 form ask for From I-551 from and Form I-797. I don't know what are those. Can I just send copy of green card? Also, it asked for copies of passport but my passport was lost( I still have the copy though), Do I need send the passport copy to INS?

You don't need a passport to apply for a Reentry Permit.Form I-551 they're talking about is your GC and form I-797 is you GC approval letter.If you have your GC,you don't need an approval letter any more.Just send copy(Both side) of your GC. If You have not yet your GC,then send copy of your approval letter(I- 797).
