How to apply for AP

Hi
I am in chicago, my case was filed in CSC center, i got AP, EAD renewed twice through my attorney.
This time i wanted to apply my self, i want you guys help.
First i want to apply for AP, as my EAD expires in June i can wait for another 2 months, please help me for AP

I want to know
What form to download
What things to attach
How much amount to attach (personal check? or what?)
Where to send

Please help me in this.
