how to apply a re-entry permit?

Hi Guys,

I just got my PP stamped on Dec 27 2004. Now I have to go back to my home country and may stay there for more than 6 month due to some personal reason. Can anyone tell me how to apply a re-entry permit? What documents do I need to submit? Is there a problem if I leave US right after the PP stamping?
