How long is it taking for AP and what are the documnets required?

AP took 30 days to get approved.
applied on june 15,03 & got approved by july 15,03

I am applying AP for spouse who has ead.
I am thinking of sending following documents, but wants to clarify with you guys,

1. Application
2. Check for $110
3. EAD Card(do we need it)
4. 2 ADIT Photoes.
5. Cover letter
6. I-94



Do we need to send a copy passport, copy of I-140?

Am I missing something else. . .
