how good is my score?

i got a 92/228 score in step 1.i am aiming for an internal medicine residency in the mid west starting in 2003.i am an IMG.how good are my chances?
i am taking my step 2 in dec.

How do you plan to come to USA for the CSA? I heard they do not give visas from India to enter USA to attend the exam?
