How do I Give up Green Card?

I have had the green card for about 15 years, but have been living out of the US for almost 5 years (I go back every year). I no longer think I will be moving back to the US so I want to give up my green card so I stop having to make the trip every 6 months to maintain status. Does anyone know how to give the green card and if I can do it from abroad??

Also, is it difficult to get a visitor visa if i give up my green card?
 
Thanks Joe! Do you know where I can download the I-407 form they talk about?..I can't seem to find it on their site.
