Ok I just downloaded the N-400 Form. I am married to a US Citizen and obviously I am trying to become a citizen.
It ask under Part 1 - C .. If you ever used any other Names provide them below.
Does that mean I have to put my Maiden Name in there or should I leave it blank which I think I shouldn't but I just thought I ask in advance !

If you used your maiden name (in your old passport, or any where else), yes, put that down there.

Please also read the excellent instructions provided on how to fill up the form. It answers all these questions.

Yes, as pointed out by Shribuy, if you have used your maiden name in your passport (current or old) or for school enrollment or any other place that needed you to record you name, you will then have to put that down under "Other Name" field. And, btw, based on feedback from several other folks who have gone through the naturalization process, this not at all an issue. We had the exact same question when we were filing our N-400 application.

I just looked at my Passport and under Name it says at first my current last name and underneath it says by birth and then my maiden Name so yes put the Maiden Name under other Names right ?
