help with I-485 filing

I filed I-140 petition in Extraordinary Ability a month ago and plan to file I-485 along with EAD and AP applications now. I wonder if anyone who has prior experience can post a list of documents (including forms and supporting documents) that I would need to submit along with the application (for I-485, I-765, and I-131 together). I would greatly appreciate it. BTW, my background is as follows: F-1 in a big ten school, Ph.D. in 2001, then work under H-1B until present, married without kid.

Form 9003, I-134, I-864...

Thanks a bunch. Just want to make sure. Form 9003, I-134, I-864 are not required if I file employment-based I-485, correct?

What about tax returns?

Finally, is signature card still required for EAD application?

I think I-134 is necessary if you have dependants. I needed for my CP interview last year.
