Help with form I-765

My AOS application is almost ready, I just have little concern regarding the I-756 application.

In 2006 I came in with a J1 visa (Exchange visitor) for 3 months and the whole process was taken care of by the program sponsor. When the visa was approved, I was authorized to work and received a SSN that was valid only with DHS Authorization.

I never received an EAD, all I got was a DS-2019 that was stamped by the inspector at the airport. After the J1 ended, I went back to back to my country.

But now I'm currently on a B1/B2 and married to a US. Citizen.

My questions are:

1)What should I check in the beginning of the I765 form:
a) Permission to accept employment
b) Replacement (I know it's not this one)
c) Renewal of my permission to accept employment

2. What should I check on Item #11 "Have you ever before applied for employment authorization from USCIS? When it asks to which USCIS office I had previously applied for my EAD, and the date, which date is it? Is it the beginning of my OPT? or is it the date when I sent my application?

3. Should I attach copies of this documents (visa, ssn, DS-2019) or a letter explaining this?

You're help will be much appreciated, I'm hoping to have everything ready by monday. :D

I didn't get it. Did you ever SUBMITTED application for 765? If you did you at least should receive "Notice of Action" or something. J1 is just a J1, it has NOTHING to do with EAD. So just check "No" in question ".. applied for EAD in US...?"
 
No, I never submitted an I-765 even tho I was authorized to work and got a SSN. I did get a notice of action, but it was pertaining the J1 visa.

So I guess this has nothing to do with my AOS application. I just wanted to make sure.
