Help with DS 230 Q10 and 11

Hello I`m prepareing the ds 230 form and have some questions, I want to make sure before sending it,
Q 2 other names should I put NONE or N/A and on q3 should I write my name again or just put N/A?
question 10 where ask about a permanent address in the US can I put: YET TO BE DETERMINED, because I don`t know it?
In Q 11, It ask for an address in the us where I want my Green Card mailed, Can I put a Friend`s address?

N/A (but NONE would probably be fine too)

N/A (assuming your native language uses roman letters)

Yes you can (I put NOT YET KNOWN, which is basically the same thing)
