HELP! Supp. Form to I-693 - Vaccination Waiver?

Hello!

I am so glad to have found this forum, and I only hope that someone can help me!

I received a notice (three actually - myself + 2 children) from USCIS requesting "additional evidence" for my I-485. It says I have to submit a properly completed "supplemental Form to I-693, Adjustment of Status Applicant's Documentation of Immunization."

We had our medical examinations and the physician filled out a Vaccination Documentation Worksheet, of which I have a copy in front of me. Is this the supplemental form to I-693? Our vaccination records are incomplete and it states on this document "Applicant will request an individual waiver based on religious or moral conviction." Um, if I need to do that, how do I go about it? Do I have to see another physician...even though we already saw one?

Hello!

Lack of required vaccinations is medical grounds of inadmissibility.
right now you can either go back to the doctor and get those vaccinations done and filed properly with INS on supplemental Form to I-693 (Vaccination Worksheet), or go ahead and file Form I-601,Application for Waiver of Grounds of Excludability based on your religious or moral conviction, including $250 payment fee (for each of you).
You can file your and your children wavers yourself, no need for another doctor’s exam.
