Help required on Form B

Hi,

I am applying for myself + wife + 2 kids

since we have to attach Form B (in duplicate) for each applicant., Does the information on Form B be only for that particular applicant?

for Example ,
On #19, should we only gie info about the applicant to which the Form B is going to be attached to or all the applicants?

On #20, it says "Previous nationality of applicant(s)"
Do i write name and nationality for each 4 of us or only the one to which the Form B is going to be attached to?

Another question - For #24 for kids - do we have to sign at this place (or should we have thumb impression of hte kid) along with the signature at bottom for "Application for minors"?

I gave info of all people where applicable. wat is question #19? i do not have the photocopy handy.
For previous nationality of applicants I gave

1. applicant1: by naturalization, date

• applicant2: by naturalization, date

• applicant3: by birth DOB

signature, my and wife signed
