Help on filing FOIA

Gurus,

I have few doubts while filling the G-639 form to get copy of my labor certification and I-140 related documents.

1. Do I need to fill section 3 "Consent to Relate Information"?

2. Under section 5 Data Needed on Subject of Record.
Name of other family members that may appear on requested records(i.e spouses, daughter, son);;;
Do we need to fill this section?

Whose Father's name?

3. Section 8 of Notary?? Do we need to notarize?

4. My application was filed in NSC. Is this correct to address to send

i think "consent to release information"
only if you are not the applicant. if you want to see somebody else's informtiaon. you do not need to fill it. i did not fill.

the info about fathers name...etc i do not think you need to. just fill your name details. give any of your petition numbers that you have (mostly 485 that is what ppl seem to be doing here)

i did notarize...just in case...who knows. even though it is not needed. i did.

i think the address is correct. but my case is in VSC someone suggested sedning it directly to VSC...which i did. i sent it thru certified mail with acknowledgement. i did recv the acknowledgement.
