Help on Affidavit of support for Visitor visa

Hello:

My mother had applied for her visitor visa through drop box and received a letetr back stating that she needs a completed affidavit of support. I couldnt find any information missing. Can someone tell me what needs to be filled in section11 of form I-134 and also any other advice that you can recommend

Looks like either you have not provided her a I-134 or it has
been sent with partial information.

For the Item # 11:

Tick the box for : I intend.

and then mention as follows:

"100% cost of lodging, food, travel and medical expenses for
a period of 6 months".
