Here is what I did:
Go to INS site:
http://www.ins.usdoj.gov/graphics/formsfee/forms/index.htm
Locate I-765, download and print it on a laser printer (it is in pdf format).
Fill-in the form (just one page), Mail it to appropriate INS office (details are on the form instructions) with the following documentation.
- 2 photos (according to specifications in the form instructions).
- the tear-off portion of the previous EAD card (It is in the letter you got with the EAD).
- Photocopy of the current EAD (both back and front).
- Photocopy of I-94 (Departure record, both back and front).
- A small typed letter stating all the photocopies are real photocopies from the original documents without any re-touch or modifications (sign at the bottom).
- photocopy of your I-485 recript notice.
- check for $100 (or money order)
