please explain "bust"

What do you mean by I-140 got "busted"? Do you mean denied? If so, why? I know I-140s have become really slow, but didn't realize they were getting denied. Would appreciate more details (e.g. EB2/EB?, Substitution/own LC? Concurrent?, RFEs, dates?)
