I have approved I 130 based my wife and now just she become a Citizen but waiting for Oath ceremony letter...She has applied for Name change with her citizenship so it may take time, so is there any way to upgrade my file ? can I file adjustment of status or not? My I 130 was applied on OCT 2010 and it was approved on March 2011.... still waiting for priority dates... is there any idea how do I maintain or upgrade my status?

She's not a citizen yet ... she must complete the oath ceremony to become a citizen.

She has applied for Name change with her citizenship so it may take time, so is there any way to upgrade my file ? can I file adjustment of status or not? You'll have to wait until she completes the oath, then you can file I-485 ASAP with a copy of her naturalization certificate and they will upgrade the petition to reflect her citizenship status. After your OPT expires you'll have a 60-day grace period to continue staying in the US, so if she completes the oath by late April you'll be fine.

You should go ahead and schedule the I-693 medical appointment with a designated civil surgeon soon, so you'll get back the envelope with the results and have it ready to send with the I-485. The medical can be done as much as 12 months before filing the I-485.
