Help! is H-4 visa multiple?

Hi All,
I am working on H-1B (valid to Oct/15/2005), My wife is studying in Canada and got her H-4 last Dec. This Jan she came to US to visit me, and after that she went back to Canada to continue study.
My question is that: if she will come to US this Apr again, she needs to apply a new H-4 visa again? or she can directly fly to US?

Does the "PED 15OCT2005" means she can come to US multiple times before Oct/15/2005? If it is, what does the "Entries 1" mean?

Entries should have been M. (for multiple)
Just check whether the answer to any of the questions are M
Generally you specify that you want multiple entry visa in your cover letter.
She has to reapply for H4 again in any US consulate.
