help in filling I-130

Am filing I-130 for father
his passport has name as follows: Surname: name1 name2
given name: name3
am i correct in interpreting it as: name3(first) name2(middle) and
 name1(last)

or is last name and no middle name.

Any help appreciated. Thanks in advance
