Help, I-693 Question

I am filing I-485/I-130 applications for my wife who is on H-1B visa. I am a USC. I have a question on I-693 form that is required with I-485 packet.

- Does this form needs to go in the same packet to Chicago Lockbox?

- Where do I find a list of doctors who do such work?

- Will they give my wife results in sealed envelope?

I would appreciate some guidence.

Yes it needs to go in the same packet with everything else.

The list of civil surgeons is on the USCIS website.

Yes, they will give you the results in a sealed envelope.
