You should find a job first. You will have to do a residency or a fellowship before you get a job. And you need your USMLEs before you get a residency. Residency programs will sponsor you for a J1 or a H1 visa. J1 is easier to get but you have to return to your home country for two years after finishing your training.

thank you for the reply

Whatever you do, DON't mention to any consular official or border official that
- you are a physician
- you are unemployed
- you intend to practice medicine in the US at some point in your life

(this makes you an 'ineligible alien physician' and excludes you from entering the US until you have passed your USMLE exam)

Whenever you apply for a visitors or other nonimmigrant visa to the US you have to proove that you have ties to the country you currently reside in

- family that stays behind
- a job (bring pay-stubs or tax records)
- an unabandoned residence (rent or mortgage receipts)
- you have the financial means to pay for the trip without having to resort to work

Otherwise they might think that you are trying to stay in the US to work illegally.

If you indeed want to work in the US, this is what you have to do:

- pass USMLE 1,2ck,2cs
- find a residency position
- obtain a work (h1b) or exchange visitor (J1) visa
