Help Ds230 Form

Hello,
When we sent our packet3, I had filled out OF230 form. But, now the consulate has sent blank DS230 form back with packet4. What should I do?
Should I fill out DS230 now or OF230?

Also, in OF230 when they asked for present address, I had mentioned my US address. However, I heard that I should have written India address (parents address). What should I don in DS230? Should I write the India address or should I stick to what I wrote in OF230?

Use DS230

The consulate will accept either of the forms but DS230 is the up-to-date form. Use that. Fill that and take it to the interview. You can update any new information that you didnt fill in the PKT 3 time frame. Present address should be the US address.

send ds form you will be fine. dont worry about packet3 now. that is past once when you get the interview date and mention your us address in the present column
