Purchase you medical insurance with american express.See any american express office in your city. You may also visit them @ www.americanexpress.com.I bought mine with them here in New York city for my french visa and it cost me $38 for 9 days, with $ 50,000 coverage. The price goes by the lengh of time you want ot spend overseas.I don't know how much is gonna be for 6 months.I will advice you to apply for a short stay visa for like 7 days and pay less for your insurance.Once you're in Italy, nobody is gonna deporte you to the US because your visa has expired.
Good luck

I can't help you with the ins question but don't make the mistake of staying, or worse working in Italy with expired visa.. it is not like here they will deport you and never be able to get a visa from them again

Ofcourse samoel is right. Don't do anything illegal in any country. Being accepted in a country without a visa(when visa is required) is different than staying on an expired visa..
