H4 to J1

Unfortunate but true
I know it should be avoided but... I want to know what is the procedure for converting H4 to J1(not eligible for H1) residency while staying in USA. Can the person start residency as soon as ECFMG issues IAP66 ? I heard you have to send it to INS for AOS to J1. Going out of US is not an option at this point because of lack of time
