H1b visa transfer

What if a person holding a H1B for Company 'A', applies for an H1B transfer through Company 'B', and once the transfer is approved, he changes his mind and doesn't join this company 'B'. Can he continue to work for his original company 'A'?
What are the consequences and precautions I need to take.

If original company accepts you back!Yes then you won't have a problem but your intentions and reason for transfer and cancellation would be questioned and proper valid explanation would be sort.
 
H1B not transferred before joiing

What happens if the H1B is not transferred prior to joining a new company? The new company after the person joined has started the process. Meanwhile should the candidate leave the country? Also should the visa be restamped to enter?
