H1B visa extention and green card through H1B

Hello everybody!
If anybody had such experience or just knows how to handle such matters in common or in details, please give an advise what to do.

My husband has H1B visa and I have H4, it will expire in 1 year.
How can he extend his visa? What is the process and what documents needed?
When he should start the procedure?
How can I prolong my visa? Along with my husband's visa or after his visa will be extended?

How can he apply for green card through his job if his employer agree to maintain such application?
What is the process, stages and what documents must be added?
Does he need a labor certification in this case?
Is it possible to change his specialization under visa to another one if it is corresponding to his education and previous specialization?
Are there any requirements to the annual amount of employer's income?
If his visa expires, won't be extended, but his employer applied for green card through his job, can he stay inside US and continue to work? How can he stay in such a case inside the country? Can he receive work permit, how?

Read the USCIS website on employment based immigration. You need to understand the he cannot initiate the process except with exceptional qualifications. His employer must do so and the requirements are very strict and difficult to meet witht he current unemployment rate for UScitizens and current permanet residents.
