H1B Visa expired

Hi Friends,
My H1B visa on my passport is expiring in a month. My h1 is valid till may 2004. I wnt to get my visa revalidated.
I was filling the DS-156 form and there is a question

36. Has Anyone Ever Filed an Immigrant Visa Petition on Your Behalf?

What should i answer ?
My I485 was filed last year in august.

I would really appreciate if someone can answer my question?

Say Yes

Reply in the affirmative. I think the form also asks for your A# which you should provide.

I did the same last year and the validation went through without a hitch.
 
It does not ask for the A#.

If Yes, Who?

For the second part of the question should I mention my company name.

I would appreciate if you clarify.
