H1b Transfer and GC filing Query

Hello. I am a therapist working on an H1b visa for a nursing home A. My H1b started on Jan 1, 2007 and it has been extended untill OCT 1 2012.My PERM and I140 has been approved under EB3 category PD 1 MAR 2009. I would like to change my job as I am interested in working full time for a hospital B.

My questions are :
1: Is it safe for me to change my job at this point? Do I have enough time for an H1B transfer and restart my GC filing again(hoping in EB2 category this time)?

2:I am also interested in continuing to work at my previous job A on an on call basis (4-5 hrs/week depending on how well they need me). What changes do I require with my immigration status. Should I transfer my H1B or ask the hospital B to file for a fresh H1b (I have a Masters degree from US)

3: What will happen to my previously approved PERM and I140 approval?

I would deeply appreciate if a kind soul can answer to my questions as I am bit concerned. Thanks
