H1B situation

I have:
- F1 visa that will expire on 08/15/09
- a pre completion OPT that expires on January 2010
(I couldnt do a post completion OPT at the time because I only did the 30 credits I was required to do in order to have my transcripts equivalent in the US for Physical Therapy so I could start the process for my license with the Arkansas State Board of Physical Therapy - which now I only need to pass the bar exam)

I am working as a Certified Nurse Assistant (while preparing myself for my physical therapy bar exam) because I need to support myself. I have the CNA license in Arkansas and now I am transferring it so I can have in Texas too.
My employer at Cornerstone Retirement Community (Texarkana) and filled out the form for the H1B1 visa.
I gathered my documentation for it (photocopies of my I94, I20 (the one used to do the F1 and the one used to do the OPT , the OPT card, my Visa and Passport - and, a copy of my trasncripts that prove that I have a bachelor's degree and teh copy of my CNA license)
I gathered the 3 fees: $320 for the H1B petition + $500 fraud fee + $1500 for the training fee since Cornertone isnt connected to an education organization-- they are a religious non profit organization though)

I know that with an expired F1 but valid OPT I can stay in teh USA just fine.
I know I get in the cap that was 44500 or something like that in July so I am still in.

but I have some questions...

since I am already working with my OPT and I will apply for the H1B... they say that when you apply for an H1B you have to start working on october 1st but I am already working since I have a job authorization... I just need to change teh status.
so I can keep working right?

are those documentations enough?
whould I have something else form my employer?

my backup plan is to ask for the 17 month extesion on my OPT but doesnt it have to be post completion?
