H1B restamping Canada/Mexico, Help Please..

Here is my situation.. and questions are after this

Got my first H1B approval May-2002
Has B.E. Electronics from India
Came to US in Apr-2003
Visa expired in May-2005, I-94 expired on the same date
Got second approval from May-2005 to May-2008
Joined MBA(Fulltime) in Aug-2004, and continued to work with same company
Got Canadian Visa (PR) in Feb-2006, can go to Canada before Oct-2006
Finished MBA in May-2006
Got another job based on MBA starting from 15-Jun-2006
Want to go to Canada for multiple entry before OCt-2006

1) I have accepted a new offer (based on MBA) from new employer
employer would transfer my H1
should I submit expired I-94 with the application?
what about wife's I-94?
when can I start working with the new firm?

2) I came late by 10 months on my first h1 visa
when should I apply for capturing this 10 months to extend my stay accordingly? can i ask my new employer to do that?

3) I am planning to go to Mexico or Canada for stamping
My name is Sanjay and I heard (not sure) that rejection rate is higher for people of this name. is this really concern?
In that case I would like to go to Brownsville, let me know if there is any usch conern over there.

4) while stamping, what should be the status of I-94? should i have the valid I-94? if yes, how to get the new I-94 for me and my wife?
my I-94 (including my wife) expired in May-2005 and my approval is upto May-2008
