I'm planning to send my passport and documents for H1B renewal. What do I need to fill in form DS-156 in the following entries?
1) Home address, U.S. address or India address ?
2) Home Phone number, US # or India #?
3) When do you intend to arrive in US?
4) Who will pay for your trip?

thanks for your answers..another question..
In form DS-156, there is a question-
"Has anyone ever filed an immigrant visa petition on your behalf?"
1) Should I answer Yes ? (since my spouse's company has filed for my GC )
2) If yes, should I specify my spouse's name or his company name?
