Legally, you need to file one day before your h1 status expire, and then you will get ~ 240 days for grace period for H1 petetion to be apporved. But in between if you need to travel outside country you will be stuck, (there is no AP for h1 adjusty)
After introducing premium processing regular processing takes long time, but mine came back within 3 months from NSC. (in aug'2002) And I would advise you not take chance and start process for h1 ext ASAP and well before your h1 expire, otherwise there is no need to primium processing although it very fast. (if you employee ready to pay i would go with that option)
