H1b cap exempt to H1b cap subject

Hi! I'm a resident with a current H1b cap exempt expiring on June 30, 2010. I am being employed by a cap-subject hospital. I was told that I can do an H1b transfer and be allowed to work even after my current H1b expires, and while waiting for my other H1b to get approved (which will only take effect on Oct 1st, 2010). I was also told that by September, I will need to go to my home country for change of status. Is this true? Why do I need to change status if it will still be H1b? Is there any other options except from going back to my country?
