H1b approved, now what?

I graduated in the medical field last year and have been working on my OPT for a doctor in his practice. I took my lawyers advice at the time last year and we started the PERM cert process as she advised me that would be the best and simplest way for me to get my GC without having to be tied down too long to one employer. The PERM cert process did not work out due to various reasons and so we filed for H1b this year and I received the approval notification letter on Monday. My OPT expires June 7th 2009 and according to the letter the H1b is only valid for an October 1st 2009 start date. My employer still wants me to work, my lawyer has advised me that I can volunteer my services but not expect to be paid. I do not know what to do, as it may be risky to continue working.
Any suggestions on how to handle this?
Also, if I get married in the next year or two, what would be the best way for my spouse to move here?
 
Believe it or not, you are not even allowed to volunteer. Your lawyer, if she's competent, should know that. You need a working visa when you work or you stay home. If you volunteer for your employer, you can face consequences if the USCIS hears about it.
