H1B approval steps-please clarify

Hi,

My wife's employer applied for her H1B 3 weeks back.

I have 2 questions:
1- It seems this is a two step process. First a labor condition approval (LCA) is required and then one can file for H1B with approved LCA. Is this true? Also, If an LCA was applied for in 1st week of Feb in NJ, when can one expect the approval?

2- Once LCA is approved, is it pretty much guaranteed that the person will get the H1B status or is there still a significant possibility of rejection?

LCA should be a matter of days not weeks.
LCA does not guarantee at all the approval of H1B.
