H1 Visa Revalidtion

Hi,

I planning on applying for revalidation of H1 visa and am having
problems filling the DS-157 and DS-156 forms. Can you pls advise.

Also have the following questions.

8) Full Name and addr of contact person or org in USA:
   What should I enter here, company addr or friends addr

DS-156

12) Home addr ? ( Should I enter India or US addr )
13) Home telephone ? ( India or US )
19) What is the purpose of your trip ? ( No trip planned)
20) At what address will you stay in the US? (Already in US and working)
21) When do you intend to arrive in the US? (Already in US and working)
22) How long do you intend to stay in the US? (Already in US and working)
23) Who will pay you for your trip? (Already in US and working)
27) Have you ever been in the US ( When, how long )?
    Should I mention differnt dates or just years
 
28) Have u ever been issued a visa ? when and where and what type
   I had 2 visa type ( F1 student visa and H1 visa) . Which one should I mention.

31) Has anyone ever filed an immigrant visa petition on your behalf? (Don\'t know )

Also, I am planning on sending the following docs
1) DS-156
2) DS-157
3) I-797 (original )
4) 2 photos
5) passport ( original)
6) Employer letter
7) Letter with contents
8) Fedex return envelope.

Do I need to send some security form ?

Am I missing anything. Pls advise.

for ds 157 you have to write your own name and address if you are in US.
for ds 156

home address (indian address)
where you are going to stay - your US address
purpose of the trip - temporary assignment
how long will you stay- look at your approval notice put the last date of the same
who will pay for the trip - self
have you been to US - If you are in US then say yes and how long will be the time period of your last visa
have you been issued a visa - that would be your last visa issued which place and when
has the company where you are working applied for your green card if yes then yes else no

Akhil , I just wanted to know in what circumstances we get our visa revalidated. I was reading on State deptt web site, there it is mentioned that one has to give reason.( applicable to both H1 and L1)
So what reason I can put in applications ? e.g. ????

I am planning to apply for L1B revalidation. I have to give reasons for visa revalidation in the US and a letter from employer.
Please suggest what reason should I give and what employer should mention in his letter.
