H1 Revalidation in the US ( form DS-157 )

Hi friends,

My H1B VISA expired on Feb,2001.My employer renewed the VISA and I got the H1 notice of action.Now I have to go for VISA Revalidation at St:lous,MO.

I have the form DS-156 to apply.I heard that DOS introduced a new form called DS-157. Is it necessary for VISA revalidation in US?
Is this form available anywhere?I checked the www.travel.state.gov,but it is not there..

Go to Murthy.com. click on updated immigration news. then you will find a discussion about the form. download from there.
 
Not so sure ...

The US State Department web-site clearly mentions the words - "Effective immediately" in the beginning of the 2nd paragraph:

One more thing - the images or pdf files included in the links posted are just scanned copies of the printed forms available at the consulates - it is not the original one like the one for DS-156 provided by the State Department here:

It is not yet available at DOS website !!

Is it necessary for revalidation?
