H1 Revalidation Form Question

Hi,

I am filling DS-156 for for H1 revalidation. I have some question on that.

Please help.

1. My wife is on H4 ... do I need to fill seperate forms for her ? Do I need to send fee for her ? (150 for me and 150 for her )

2. In that form do I need to provide my India Address ? and also what should I put in column for date of entry in USA ( I am already in USA)

well yes, there has to be seperate form for each person that you are sending passports for revalidation. Same goes for fee, though you can send it as one check. ($150x # of persons).


As per the latest instructions in Qs 15 u give ur US address and contact #s.
There dosent seem to be any space for Indai address. For date of arrival, leave it blank and write already in US after printing.

Also please search through the forums before asking the question, all these have been answered over and over and over again.
