H1 Revalidation Documents

hi guys, what are the documents that are needed for h1 revalidation?
i am planning to sent it to washington. my h1 expires may31, 2004. i have a new h1 till march 2006. is there a thread or link that gives all details?

how much time does it take these days for h1 re-validation from washington?
i have a US MS degree. can i try going to canada for h1 revalidation. i have a h1 stamped through washington before.

is it risky to go to canada?

In DS-156 question number 36....

I was with employer 1 and now with employer 2.
I am trying to stamp my h1 from employer 2 in my passport.

employer 1 has already applied for my gc and its in the process (140 approved just now and 485 pending)

what should i answer for question 36? (Has anyone ever filed an immigrant visa petition on yoru behalf)...if i answer yes, i might get issues with non-immigrant visa stamping. if i say no, i am not giving correct information.

please advice.

is the following enough or do i have to send copies of w2, tax and paystubs?

1] My original Passport

2] Original Form I-94 (stapled with the passport)

3] Valid Form I-797

4] Comprehensive letter from my current employer, identifying me and describing in detail the nature and function of the position, the kind of business and justifying the need for the visa.

5] Typed nonimmigrant visa application form, DS-156 (with one passport size photo, stapled)

6] Typed supplemental nonimmigrant visa application form, DS-157.

7] $100 money order, made to “U.S. Department of State”

8] $50 money order, made to “U.S. Department of State”

9] Pre-paid envelope (Federal Express), for returning my passport, after stamping the visa.
 
Is the visa revalidation in US done at Washington or St.Louis?
Also, I heard from my attorney that for REVALIDATION WITHIN US, Dept. of State is not accepting applications after Julu 1st.
Any input from all Ginnu / other gurus will be greatly appreciated.

The revalidation application is received at St. Louis for initial scrutiny and payment receipts, eventually that application is forwarded to Washington, DC and DoS at Washington, DC sends the stamped passports back to the applicant.

The discontinuation of revalidation in US starting Sept 2004 is NOT officially confirmed by DoS yet.
