H1 premium processing questions

A friend of mine is looking to apply for her h-1 extension and had a few questions:

1. What are the documents that are required to process an h1 in premium processing?

2. Is it true that a regular h1 cannot be renewed before 6 months of the previous expiration?

Can someone point me to any websites/faq's on these issues.
