H1 extension denied

1) From when a person becomes out of status once the H1 extension has been denied - from the date of previous H1 expiry or from the date of denial?

2) Can H1 to H4 transfer be filed AFTER the H1 extension has been denied?

3) If not, would it be a problem to get H4 stamped in the native country or the third country post?
