Go to travel.state.gov, which is a state department site, and click on visa for foreign citizens. This will give you all the information that you need.

You can get visa by staying in the united states if you meet the following requirements:

  1. You have valid H-1 visa on your passport that is not expired for more than a year. The visa doesn\'t have to be from the same employer.

As pointed out, go to travel.state.gov for the address to which yu can send your H1 stamping application. on INS site, there is a form which you can fill out in HTML. It gets converted to pdf using some horrible code by INS (the result is that the ouput may not be what you wrote). In that case, adjust the HTML form appropriately and fill the rest by hand.

You need the 797 (or, the first page of your H1 approval), a valid I-94 and a letter from your employer requesting for the stamping.

The fee is $50 and $75 extra for Indians (for flocking to the US, it seems). You can FedEx or use US mail. The addresses for each are different.

You can expect your passport back in about 3 weeks.
