H1 B visa revalidation within USA.

I am filling the DS 156 and DS 157 forms for the visa revalidation. Can somebody please tell me for DS 156 form column 10, what does National Identification Number mean for Indian Nationals.
 
Employment verification letter.

Does anyone has a format for employment verification letter we need to send with the application for H1 B visa revalidation?

I had left the National Identification Number as blank (OR) I had written N/A. I cannot remember.

I sent it out around March 15th and got it stamped around May 9th.
 
Another query.

Thanks for your response. Can you please tell me did you also send your original I 94 (which we get when we enter US) or just sent the one which comes with the H1B petition and what type of letter of Employment did you get from your employer (I mean the format of the letter). Also should I file for revalidation through my company\'s lawyer firm or should I file it independently (which one is better)?

You can find sample employment letter on www.immihelp.com in visa re validation section. You can file by yourself. You don\'t need lawyer for that. You need to send both I-94. And send through fedex, not through usps.
