H1-B Stamping in Chennai. Info on Reqd: Documents, Tips/Suggestions solicited...

Hello all,
I am planning to goto Chennai in Oct,
I have a valid H1-B, though not stamped on passport yet.
(Previously stamped F1 has expired.)

I need tips/suggestions that you can provide me in the process of appearing for the interview for multiple entry stamping...

A: Also, has any one who went to chennai been rejected?

B: If so under what circumstances???

C: Was a reapplication honored favorably? If so what remedy was followed?

D: I understand the following are the documents you need.,
Please correct me if i am wrong.

0. Secure an appointment at the embassy.
1. Valid H1-B papers...(I-129?) Copy of all previous H1-B, I-94 etc...
2. Passport
3. 'Visa Photo' (how many???can i take it to INS specs in California itself???)
4. Form DS156 (and DS157 if male, 25<age<50??)
5. Two DDs...(dont remember the amounts...)
5. Letter from Employer (Is it an over kill to get recommendation letters from manager/VP???)
6. 3 to 6 months of Bank Statements
7. 2 to 3 years of tax filing. (I have only 2 years, the 99 one i dont think i have...must have lost it., is 2 years okay???)
8. All University degrees and transcipts. (12'th scores i have...am not sure about 10'th...is that needed???)
9. A brochure on what the company does...
10. Photo of the applicant in his/her office, and outside the company building.

anything more...???

E: Can anyone in the same situation, (that is F1 in passport, out of school with valid H1-B, havent travelled out of country yet, and so needs to get passport stamped with visa, AND been to chennai consulate)...share your experiences here??? what were the general mode of questions in the interview...How long does it take to get pp back in mail...
