H1 at St. Louis and H4 at chennai ??

My wife is already in India and has to go to chennai for H4 stamping. But, I would like to get my H1 stamping done here at St. Louis.

Is it possible to do this? If so, then what do I fill in the following columns:
25: Name of person who you will be staying with?
34: Names and relationships of persons travelling with you.
37: Are any of the following in US?
