H-4 visa requirements (urgent please)

I got married when I went to India. I am on H-1 visa and would like to get H-4 visa for my wife. Can some body please let me know:

(a) A list of supporting documentation I need for the H-4 visa.

(b) What do you mean by the statement : "Principal applicant's name should be included in the passport, either on the final page under "spouse's name" or as an annotation by the passport office ? Is it essential ? How do I get it done ? How long does it take ?

I have appointment on June 9th. Please respond quickly.

a) Go through the forums, there are lots of posts describing the same.
b) Go to the PP office and submit the spouse's PP to have you name in it. Again these forums have all the necessary info.
