H-4 Extension

My wife entered US 2 weeks ago with H-4 status. But she didn't carry my H-1B supporting documents (My H-1B is valid till August 2006), therefore the custom officer stamped her I-94 with valid period to 12/8/04 and ask her to go to the download office to extend it. Since she doesn't speak english well, I am not sure whether that is accurate. Do I have to file the formal application forms to CSC to extend here H-4 or I can just go to local San Jose office to extend? I will appreciate your quick responses.
